2010 Fitness Model Calendar
CAN YOU QUALIFY FOR THE NEW
FITNESS MODEL CALENDAR
AND
POSTER?
In order to be considered,
you must:
1. Send THREE color photos of yourself
in a bikini
(photos will not be returned unless you provide a SASE)
2. Provide your vital information
(a) Full name
(b) Complete mailing address
(c) Telephone number(s)
(d) E-Mail address
(e) Age, Height, Weight, Eye Color, Hair Color, Measurements
(f) Will you photograph topless? Implied Nudity? Full Nudity?
(these are NOT requirements, just options for the publisher)
3. Enclose your application fee of US $15.00,
payable to: Kevin Weaver Fitness Productions

by money-order or bank/teller check
(neither personal checks nor cash will be accepted)
4. Only 13 models will be selected.
If you are selected, you MUST be able to travel at your own expense
to Las Vegas, NV during the first week of April 2009
for the PHOTO SHOOT.
(Also, you are responsible for your own meals and lodging)
5. You will be given a CD of your photos for your personal
use.
6. When the calendar and poster are published, you will
receive
ten (10) free copies of each.
